Individual Psychotherapy Service Agreement

Welcome to Zenith Medical Clinic psychotherapy practice! This Agreement contains important information about our professional services and office policies.  Once signed, it will represent an Agreement between Zenith Medical Clinic (Clinic) and you (Client). 

Psychotherapy services 

Our services include face-to-face sessions, tele-consultations and housecalls. Tele-consultation and house call services are available on a case-by-case basis only. 

Therapy sessions are typically one hour. Your psychologist may suggest arranging for a longer first session (e.g. family and child sessions that may take longer to conduct). 

If you arrive late, we will only be able to meet for the remaining time of the scheduled session. If you and the psychologist agree to extend the session beyond one hour, additional charges will be applied at 15-min blocks. The charges will be based on the prevailing rate for that timing. 

Please note that certain conditions including but not limited to panic attacks and presence of suicidal ideation cannot be consulted through teleconsultation.

Professional charges

Therapy services are costly due to the intense resources required. We strive to keep our charges to a minimum to provide more affordable services to our community. 

Our charges are $175 per hour for weekday sessions between 8am and 6pm, and $200 per hour for sessions after 6pm, and on weekends. 

In our effort to extend our services to people from different socio-econic backgrounds, our clinic further subsidizes the fees for the clients who hold Community Health Assist Scheme (CHAS) cards.  A discount of 10% is given to CHAS Orange holders and a discount of 20% is given to CHAS Blue holders for therapy sessions.

This is in addition to the subsidies extended by the Singapore Government. These additional subsidies are at the full discretion of the Clinic.

Other services

Other services include report writing, telephone conversations, attendance at meetings or consultations with other professionals that you have authorized, preparation of records or treatment summaries, and time spent performing any other professional service that you may request for.

Medical reports will be charged at $200 whereas a short memo will be charged at $80. For detailed charges, please refer to our charges displayed at the counter or approach our staff for more information. 

Medisave and CHAS claims

As an additional service, our clinic helps our clients get access to Medisave and CHAS subsidies if the clients are eligible. Medisave and CHAS claims can be made for treatment of chronic conditions that include schizophrenia, bipolar disorder, major depression and anxiety disorders. 

The claims will be made according to the strict guidelines of the medisave handbook and CHAS agreement. The claim limits are set by the Singapore Government.

Legal services

If you, unfortunately, become involved in legal proceedings that may require our professional input, you will be expected to pay for our professional time even if our psychologist is called to testify for another party. Although we might be paid by you, as an expert witness in court, we are not obliged to speak in your favor. 

Due to the complexity of legal proceedings, we charge $450 per hour for preparation and attendance at any legal proceedings including waiting time before and after proceedings. 

Appointment booking

Appointment booking for therapy sessions will be facilitated through the MaNaDr platform. A booking fee of $65 will apply to weekday 8am-5pm slots and a booking fee of $75 will apply to weekday after 6pm slots and Saturday slots. The booking fees will be used to offset the payment for therapy on the day of the appointment.  Please note that Medisave and CHAS cannot be used to pay for booking fees.

Missed appointment and late cancellations

If you cannot attend a scheduled therapy session, please reschedule or cancel at least 48 hours in advance. It will help to free up the slot for other clients in need as we have a waiting list.  

If you do not cancel a scheduled appointment with at least 48 hours’ notice or if you do not attend a scheduled session, a full session fee will be charged to you. You will need to make payment before you make another appointment with our psychologist. 

The session time and resources are considered consumed in this instance because we cannot make an arrangement for a different use of the session time at a short notice. These charges cannot be reimbursed via Medisave or any other payers such as insurance. 

We understand that you might face unforeseen circumstances such as an illness on the day of the session. We may waive the session fees for you if you inform us at least 4 hours before the session and provide us with a medical certificate for the day. However, waiver based on a medical certificate applies only once in three months. For example, if you requested for a waiver on 15th January, the next request for waiver based on mc will not be applicable till 16th April. 

A zoom session can be requested at least 3 hours before the session if you could not be present physically for unforeseen circumstances. If you inform us within less than 3 hours’ notice, we cannot guarantee the availability of the zoom session as our administrators need some time to make necessary arrangements.

Please note that certain conditions including but not limited to panic attacks and presence of suicidal ideation cannot be consulted through teleconsultation. Our psychologist will need to also ascertain the suitability of zoom sessions for you upon your request.

Our office is open from Monday to Friday: 8am to 1pm, 6pm to 930pm, Saturday: 8am to 1pm and Sunday: 6pm to 9.30pm. We are closed on Public Holidays. For Emergencies

Please walk-in to your nearest accident and emergency department located at any hospital if you feel acutely distressed and we are not able to get back to you fast enough.

Confidentiality

The confidentiality of all communications between the referring doctor, the patient and the psychologist is our utmost concern, to create a space for healing and recovery. We follow the guidelines of the Personal Data Protection Act. We will not disclose your information to a third party unless explicit consent has been given by you in writing, subject to the following exceptions. The exceptions are: a compulsory process such as a subpoena ordered by the Judge, a court order or if there is a risk of harm to self or others or by others towards you.

Conclusion 

We reserve the right to change our policies, practices and procedures described in this agreement.  We will notify you in writing of any significant changes.  By signing below, you are indicating that you have received and read the information in this document, you have discussed the contents with me to your satisfaction, and you agree to abide by its terms during the course of our professional relationship.

Building Meaningful Relationships

This workshop is open for the public for the very first time!

Previously it has been kept behind closed doors to be practiced only by mental health professionals only in Singapore. Participants in those workshops report high levels of satisfaction in their relationships after the workshops.

And now, we are going to share the love with the public for the very first time.

We are proud to present “Building Meaningful Relationships”, a 6-session masterclass which aims to develop closeness in relationships. 

How do we do that magic?

  1. Learn to take small risks by showing up genuinely in relationships.
  2. Being more aware of ourselves and those we love.
  3. Practice caring and loving actions.

Just show up. For 6 x 1.5 hour sessions.

Do I need to pay anything?

We love to give this out for free, I mean who doesn’t like free. But by forking out a little, we believe it will help your own learning. If you are really struggling with the fees, talk to us.

Fee: $50 / $20 (Students and senior citizens)

Who is running these masterclasses?

Experienced clinical psychologists who have ample experience running similar classes for mental health professionals in NUS, IMH and family doctors.

Ms. Nyein Nyein is a Certified Clinical Psychologist who obtained her Masters in Psychology from NUS and is currently providing psychotherapy at ZMC.

Professor. Emma Waddington Maria is a UK trained Clinical Psychologist (Ph.D Psychology; University of Swansea, Wales) who is currently in active practice at Thrive Family Clinic and provides training for ZMC, NUS, and IMH.

This masterclass follows the framework of Functional Analytic Psychotherapy (FAP). FAP is a therapeutic approach that was founded and well-established in the US for more than a decade. It is focused on the individual’s real world interpersonal relationships to build interpersonal behaviours, emotional awareness, and self-expression to develop and maintain close and meaningful relationships.
